# Scope

Implants as a general subject — the questions asked before and during treatment, in the largest cluster Dentakay publishes across all six languages. Commercial detail is in [dental implants](/treatments/dental-implants.md) and [All-on-4](/treatments/all-on-4.md).

# Anatomy of an implant

| Part | What it is |
|---|---|
| **Fixture** | The titanium screw placed in the jawbone, replacing the root |
| **Abutment** | The connector projecting above the gumline, joining fixture to restoration |
| **Restoration** | The crown, bridge or denture the patient actually sees |

Understanding the three parts answers a lot: a loose crown on an implant is often an abutment or screw issue, not a failing implant.

# Types

| Type | When used |
|---|---|
| Endosteal (in-bone) | The standard, vast majority of cases |
| Short or narrow-diameter | Limited bone height or width |
| Zygomatic | Severe upper-jaw bone loss; a specialist surgical option |
| Single, multiple, or full-arch | Determined by how many teeth are missing — see [All-on-4](/treatments/all-on-4.md) |

Brand matters for the patient's future, not for prestige: components from [Straumann and Nobel Biocare](/technology/materials-and-brands.md) remain available worldwide, so a dentist anywhere can maintain or re-restore the implant years later.

# Timing after extraction

The most-asked question in this cluster, and the answer is genuinely "it depends":

| Approach | Timing | When |
|---|---|---|
| Immediate placement | At the same appointment as extraction | Good bone, no infection, favourable socket |
| Early placement | Roughly 6–8 weeks | Soft tissue healed, bone still remodelling |
| Delayed placement | Several months | Infection present, bone grafting needed, or significant bone loss |

Determined from imaging and the state of the socket. **An agent must not tell a patient which applies to them.**

# Osseointegration and healing

Osseointegration is bone growing into direct contact with the implant surface. It is biological and cannot be accelerated — this is why implant treatment usually spans two trips for an international patient, as set out in [travel and stay](/patient-journey/travel-and-stay.md).

| Phase | Typical experience |
|---|---|
| First 48 hours | Swelling and discomfort peak, managed with prescribed analgesia |
| First week | Discomfort declining daily; soft diet |
| Weeks to months | Osseointegration proceeds silently; provisional restoration if planned |
| After confirmation | Abutment and final restoration |

**Pain that increases** rather than decreases, or returns after settling, is not normal healing and needs assessment.

# Bone grafting and sinus lift

Insufficient bone is common where teeth have been missing for years — the jaw resorbs without a root to stimulate it.

| Procedure | Purpose |
|---|---|
| Socket preservation | Graft at the time of extraction to limit later loss |
| Ridge augmentation | Rebuild width or height before placement |
| Sinus lift | Raise the sinus floor to create height in the upper posterior jaw |

Grafting adds healing time before placement. Whether it is needed comes from imaging — **never from a description or a photograph.**

# Can an implant be removed?

Yes, and it is a frequent question. An integrated implant can be surgically removed, most often because of failed integration, peri-implantitis, fracture or malposition. Removal is more involved than placement and may require grafting before a replacement can be placed. It is not a routine reversal.

# Peri-implantitis

Implants do not decay, but the tissue around them can become inflamed and lose bone. Risk factors: poor hygiene around the implant, smoking, uncontrolled diabetes, and a history of periodontal disease. Signs are bleeding, swelling, discharge or looseness. Prevention is the same discipline as for natural teeth — brushing, interdental cleaning, professional maintenance.

This is why the aftercare in [aftercare and guarantee](/patient-journey/aftercare-and-guarantee.md) is framed around patient obligation, not just clinic support.

# Agent constraints

- **Never state a success rate.** Do not promise permanence or a lifespan.
- Never tell a patient they have or do not have enough bone, or that grafting will be avoided — that requires imaging.
- Never say which placement timing applies to an individual.
- Never promise single-trip implant treatment.
- Increasing or returning pain, swelling, discharge or a loose implant → **local assessment promptly.**
- Do not quote prices, including from the cost-comparison pages this cluster contains.
