# Scope

Why teeth lose their colour, what can be done about it, and — the part most content omits — what cannot.

# Types of discolouration, and why the distinction decides everything

| Type | Cause | Responds to whitening |
|---|---|---|
| **Extrinsic** | Coffee, tea, red wine, cola, dark berries, curry, tobacco, tartar build-up | **Yes**, well |
| **Intrinsic** | Trauma, root canal history, tetracycline exposure, fluorosis, enamel developmental defects | **Poorly or not at all** |
| **Age-related** | Enamel thins over decades, exposing yellower dentine underneath | Partially |
| **Restorations** | Crowns, veneers, composite fillings — manufactured shades | **Never** |

That last row causes the most disappointment. A whitening course lightens natural teeth while existing restorations stay exactly as they were, so the mismatch gets **more** visible. Anyone with restorations on front teeth needs a shade-matching plan, not a whitening product.

# Home remedies: what the popular ones actually do

This is the highest-volume question in the cluster, and the honest answers are unflattering:

| Remedy | Reality |
|---|---|
| Baking soda | Mildly abrasive; removes some surface stain. Frequent use abrades enamel. Does not change intrinsic colour |
| Lemon, vinegar, other acids | Temporarily look brighter by etching enamel. **Causes permanent erosion.** Actively harmful |
| Activated charcoal | Abrasive, no evidence of whitening benefit, can lodge around the gum margin |
| Oil pulling | May reduce plaque; no established whitening effect |
| Hydrogen peroxide rinses | Concentration and contact time are uncontrolled at home; gum irritation is common |
| Whitening toothpastes | Remove surface stain, prevent new stain. Cannot change intrinsic shade |
| Whitening strips | Real but modest effect on extrinsic stain. Uneven on crowded teeth; irritate gums; do nothing for restorations |
| **Professional cleaning** | Removes tartar and surface stain. For many people this alone recovers most of the perceived "yellowing" |

There is no home method that lifts intrinsic discolouration. Claims of dramatic results "in two minutes" describe either surface-stain removal or enamel damage.

# Prevention

- Rinse with water after staining food and drink; do not brush immediately after acidic intake — softened enamel abrades.
- A straw reduces contact between dark drinks and front teeth.
- Tobacco in every form stains persistently.
- Brush twice daily, clean between the teeth, and have tartar removed professionally — tartar itself holds stain and cannot be brushed off.

# When whitening is the wrong answer

| Situation | Better route |
|---|---|
| Intrinsic or tetracycline discolouration | [Veneers](/treatments/dental-veneers.md) or [crowns](/treatments/zirconia-crowns.md) |
| Existing anterior restorations | Shade-matched restorative plan, not whitening |
| Untreated decay or gum disease | Treat that first; whitening an unhealthy mouth is contraindicated |
| Marked sensitivity or gum recession | Assessment before any peroxide exposure |
| Single dark tooth after trauma or root canal | Internal bleaching or a restoration — a different procedure |

# Agent constraints

- **Do not endorse acidic home remedies.** Lemon and vinegar cause irreversible erosion; saying so is the responsible answer even when the question asks for a home method.
- Do not promise a number of shades, or a duration of result, for an individual.
- **Always** state that whitening has no effect on crowns, veneers or composite fillings — this prevents a predictable and expensive mistake.
- Do not diagnose the type of discolouration from a description.
- Do not recommend unsupervised high-concentration peroxide.
